Our Story
CHM Mobile Care was founded to address a growing gap in healthcare delivery for medically complex patients—those who require timely, high-quality medical care but face barriers to traditional office-based and hospital-centered models. As post-acute and in-home care settings continue to expand, CHM recognized the need for a physician-led organization built specifically to deliver advanced medical services where patients live and recover.

Our model was designed by clinicians with deep experience across skilled nursing facilities, assisted living, inpatient rehabilitation, and private residences. We saw firsthand how fragmented care, limited provider access, and delayed clinical intervention contribute to avoidable hospitalizations, prolonged lengths of stay, and poor patient experiences.
CHM Mobile Care was created to change that. By bringing internal medicine, transitional care, and specialty services directly to the bedside, we support continuity across the entire post-acute and in-home continuum. Our mobile approach removes access barriers, strengthens communication between care teams, and ensures patients receive proactive, evidence-based care during their most vulnerable periods.
